PacketUpload · Form Intake

Forms that read what you upload. Kill the transcription tax.

Drop a PDF, photo, screenshot, or fax. Form Intake extracts, schema-maps, validates, and confidence-scores every field — URLA 1003, ACORD, CMS 1500, utility bills, W-2s, driver licenses, insurance cards, leases. Your users stop typing. Your ops team stops transcribing. Your funnel stops leaking.

Built for product, operations, and intake teams who are tired of paying the transcription tax.

Typing is the worst intake experience ever invented.

Every high-volume intake flow — insurance, fintech KYC, healthcare, URLA 1003, benefits, tenant screening — is secretly a transcription job. Baymard’s funnel research shows ~70% of users abandon long forms; the ones who don’t generate correction work at a 1–4% human-keying error rate.

01

Users already have the data — just not in your schema

Utility bill, license, W-2, bank statement. The facts are in the document; retyping is where funnels die.

Dropzones are receipts, not solutions

You collect a file, then burn engineering cycles on OCR, mapping, validation, and routing.

Validation on submit is three days too late

Users finish, team reviews async, corrections come back. Every round-trip is an off-ramp.

Your entry team retypes what a vendor already sent

Rossum, Hyperscience, and Ocrolus each sell you a dashboard your PM does not want to maintain.

Upload first. Type last. Or not at all.

Any document, any format. We extract, map to your schema, validate against your business rules, and populate your existing fields in seconds — with confidence scores users can confirm. No new dashboard. No re-platforming. Your backend, your form, your UX — with the transcription tax removed.

01

Upload → extract → schema-map → confirm

PDFs, scans, photos, screenshots, multi-page faxes. Extract the facts, map to your schema, populate your fields, show the user a clean “confirm this” view. Users keep moving. Your data keeps its integrity.

02

Validation at the moment of capture

Missing signatures on the lease. Insurance card expired. ID photo glared. W-2 Box 1 outside a plausible income range. Flagged the moment the user uploads — before they leave the page, before your ops team sees it.

03

Drop-in for the form you already have

JS widget, REST API, or hosted pages. Your backend, your fields, your submit flow, your analytics. We do the understanding layer. No rip-and-replace of the form builder your team standardized on last year.

Schema-aware from day one

Ships with extractors for URLA 1003, ACORD, CMS 1500, 1500 HCFA, FHIR intake, utility bills (100+ US utilities), driver’s licenses and passports (US + global), insurance cards (auto, health, dental), W-2s, 1099s, pay stubs, leases, EOBs, bank statements. Custom schemas trained in days, not months.

From a photographed form to a structured record, without a typed field.

One iPhone capture of a carrier-branded ACORD supplement. Every field extracted, labeled, normalized, and placed into the intake record in the same session the producer’s phone is still warm.

From dropzone to schema-mapped record — without the transcription tax.

Form Intake sits at the exact moment a user lands a document. We do the understanding; you keep the UX and the system of record.

Today Transcription Tax

  1. User drops a PDF into your form. You store it in S3, congratulate yourself for being paperless, and now need an engineer to build an OCR pipeline, schema mapping, validation, and a review UI.
  2. User manually re-types the same data. Your 14-field form asks for name, DOB, address, coverage dates, carrier — all already on the insurance card they just uploaded. 67% of users abandon. The rest make typos.
  3. Ops team opens every submission and fixes fields. Data entry specialists keystroke everything into Salesforce / HubSpot / Encompass / Epic. 1–4% error rate. Linear cost scaling. Backlog grows on any volume spike.

With Form Intake Upload-First

  1. User uploads; fields auto-populate in real time. Document parsed in seconds. Fields populated inside the form the user is already looking at — with inline confidence indicators where we’re not sure.
  2. User confirms; validation runs at source. Expired IDs, mismatched names, implausible amounts flagged before submit. Users correct once, on screen, while motivated — not via email three days later.
  3. Schema-mapped JSON hits your system of record. Normalized output on your schema. Salesforce, HubSpot, Encompass, Epic, Plaid, Stripe, your own backend — consumes structured data, not PDFs, not free text.

Plugs into the form builders, backends, and data warehouses you already run.

Form Intake is built to disappear into your stack. Your PMs keep their form builder. Your engineers keep their schema. Your analysts keep their warehouse.

We sit behind your form, not in front of it. Ship your hosted pages via Typeform, Jotform, Formstack, or your custom React form; wire data into Salesforce, HubSpot, Encompass, Epic, or your warehouse (Snowflake, BigQuery, Databricks). IDP layer is commoditized; the integration work to your schema is where the category has historically broken — that’s what we remove.

What PMs, ops leads, and compliance teams ask before switching.

What document types can you extract from?

Most structured and semi-structured documents: utility bills (100+ US utilities), government IDs (US + global), insurance cards (auto / health / dental / vision), W-2s, 1099s, pay stubs, bank statements, leases, medical intake forms, EOBs, CMS 1500 / HCFA, ACORD forms, URLA 1003 and mortgage declaration packages, tax returns. Custom schemas trained in days for vertical-specific formats.

How is this different from AWS Textract, Google Document AI, Rossum, or Hyperscience?

OCR services (Textract, Document AI) get you text and tables — useful, but you still own the schema mapping, validation, confidence thresholds, and review UI. IDP platforms (Rossum, Hyperscience, Ocrolus) give you a separate dashboard your PMs have to run. Form Intake sits behind your existing form, maps to your schema, validates against your rules, and returns structured data to the system of record — no separate dashboard, no new UX for users.

Can I embed this in my existing Typeform / Jotform / custom form?

Yes. Drop-in JS widget for Typeform, Jotform, Formstack, HubSpot forms, and custom React/Vue/Angular forms. REST API for fully-custom flows. Hosted pages for teams without a frontend. Bring your own schema, backend, and submit flow — we handle the understanding layer.

How do you handle PII, HIPAA, and SOC 2?

PII redaction on by default. HIPAA-eligible deployment with BAA available. SOC 2 Type II-ready architecture, region-pinned processing (US, EU, optional local deploy), encrypted at rest and in transit. Customers in healthcare, fintech, and gov programs are the baseline threat model.

What happens when the model’s confidence is low?

Per-field confidence scoring is returned with every extraction. You set the threshold — above it, fields auto-fill silently; below it, the UI asks the user to confirm or correct. Anything still ambiguous routes to your ops team’s review queue with the source document and extracted value side-by-side.

Pricing — per-page OCR, per-API-call, per-form submission?

Per-submission, with volume bands. Not per-page OCR (which punishes long documents) and not per-field (which punishes rich schemas). Pricing aligns with the value unit for intake teams: a completed form tied to a real business event.
